TL;DR

The returning 40-player raid in Dustwallow Marsh: one boss, three phases, heavy fire damage. Opens 9 Dec.
All 40 raiders need the Drakefire Amulet in their bags (it doesn't need to be equipped). The Horde chain runs through LBRS and ends in UBRS.
Stack fire resistance before the first pull. Loot is unannounced.

Location and entry

Dustwallow Marsh, level 60, 40 players, fixed size.
Each raid member must carry the Drakefire Amulet, and each player earns their own. Blizzard: "start early, bring a few friends for the elite quests, and leave plenty of time for several trips into high-level dungeons."
! Devs on the official podcast (8 Oct, via Warcraft Tavern): the attunement is not account-wide, "at least not at launch". Every alt does the chain.
Beta client: Drakefire Amulet (item 16309) is a rare neck item with required level 50.

Onyxia, Nefarian's younger sister. A three-phase fight with powerful fire attacks.
Blizzard says to build fire resistance first, and to be ready to reposition as the fight shifts.
Exact Forever mechanics and tuning: fill after 9 Dec. Don't assume the Classic fight is unchanged.

The beta client still contains Classic Onyxia items (e.g. Head of Onyxia, Onyxia Tooth Pendant, Onyxia Blood Talisman). That only proves the items exist, not that they drop: Forever reworked loot.
New Epic appearances go only to whoever the item binds to.
! Icy Veins suggests gear with bonuses against Dragonkin for this fight. Items with Dragonkin bonuses do exist in the beta client.

Blood and Thunder: Warlord Goretooth, Kargath (Badlands).
Warlord's Command: through Lower Blackrock Spire.
Report to Eitrigg (Orgrimmar), then Thrall.
For the Horde!: Warchief Rend Blackhand's head (LBRS).
What the Wind Carries, then The Champion of the Horde: find Rexxar on the roads of Desolace.
The Testament of Rexxar: Myranda the Hag, Western Plaguelands.
Oculus Illusions: Black Dragonspawn Eyes.
Emberstrife: use the Amulet of Draconic Subversion to find the drake Emberstrife in Dustwallow Marsh.
Four The Test of Skulls quests: bring Emberstrife the skulls of Scryer, Somnus, Chronalis and Axtroz. Blizzard doesn't say where they are in Forever.
Ascension...: get the Dull Drakefire Amulet, back to Rexxar.
Blood of the Black Dragon Champion: General Drakkisath, UBRS. Return to Rexxar for the amulet.

Start the chain as early as you can. It needs LBRS twice (steps 2 and 4), four named-dragon kills, and a UBRS run for Drakkisath: plan guild runs so everyone gets the Blood together.
! UBRS entry: the beta client still has the Seal of Ascension key items. Whether Forever's UBRS needs that key is unconfirmed. Fill after launch.
Fire resistance: the beta client includes Greater Fire Protection Potion and cloak fire-resistance enchants. Forever FR gear and recipes are unknown until launch.
! Plan as if world buffs get stripped on entry. That's unconfirmed.
40 Horde on the PvP ruleset, all attuned. Track attunement in #lfg.

40-player comp, tank swaps and positioning: fill after 9 Dec.
New Horde can now bring Paladins (Undead Paladin is a new combo) alongside Shaman.
